Foster Contract

Please carefully read and check the boxes next to the following statements. By your checks and electronic signature below, you agree to abide by each of these guidelines: *

I willingly volunteer to care for my foster animal I understand that the purpose of this foster relationship is solely to provide temporary care for the animal(s), and that the animal(s) belongs exclusively to RTU I agree to implement recommended training protocols as recommended by RTU I agree to return the animal the animal to RTU if requested for any reason whatsoever I understand that I must contact RTU immediately if circumstances arise in which I am unable to give the best possible care. I understand that the transfer of the animal from one foster to either another foster or adopter will only be made with authorization from RTU and that arrangements made for such transfers require authorization from RTU. I agree to make the animal available to RTU at their discretion for participation in mobile adoption presentations. Reasonable accommodations will be made to transport the animal. I understand that I will be responsible for the daily care and feeding of the foster animal. Should the animal become ill, I will contact the RTU of a Doctor of Veterinary Medicine (DVM) approved by RTU if the foster animal requires medical attention. I understand that veterinary care will be authorized solely at the discretion of the RTU staff or management. I understand that RTU cannot be responsible for charges incurred at other veterinary hospitals without prior approval. I agree that I will not administer medicine or perform any medical treatment unless approved by RTU. If medicines are prescribed, I will administer this exactly in the manner prescribed. I will not administer home remedies unless approved by RTU I understand my home may be subject to inspection I understand my own animal(s) could be exposed to medical conditions or behaviors that have not been recognized in the foster animal, and that my animal(s) may be at risk I agree that if I fail to abide by these agreements, I will not be permitted to continue to foster the animal nor foster any future animals. I understand that myself and any other members of the household, including minor children, could be exposed to behaviors that have not been recognized in the foster animal, and that we are at risk I understand that any behavioral training must be made in accordance with RTU policies I will not take my foster dog to a dog park within the first 30 days. After 30 days, I will only take my foster dog to the dog park if it is fully fenced, my foster dog has not exhibited any aggression toward other dogs or people in the past 30 days and I have received no information indicating my foster dog will be aggressive toward others at a dog park. I will not leave my dog unattended outside or in the backyard I understand that RTU will do their best to move a dog if a situation occurs where I am no longer able to care for the dog, however I will give RTU one week notice to move a dog when able. I will not surrender my foster dog to anyone outside of the organization, including another shelter. I understand that RTU can not guarantee that a dog will be moved IMMEDIATELY when requested. As a foster for Rescue the Underdog, I hereby agree that I, and my assignees, heirs, guardians, and legal representatives, will not make a claim against or sue Rescue the Underdog or its employees, agents or contractors for injury, illness or damage resulting from the negligence, whether active or passive, or other acts, however caused, by any of its officers, employees, agents, volunteers or contractors of Rescue the Underdog as a result of my fostering. I HEREBY RELEASE AND DISCHARGE RESCUE THE UNDERDOG AND ITS OFFICERS, EMPLOYEES, AGENTS, VOLUNTEERS AND CONTRACTORS FROM ALL ACTIONS, CLAIMS, OR DEMANDS THAT I, MY HEIRS, GUARDIANS, AND LEGAL REPRESENTATIVES NOW HAVE, OR MAY HAVE IN THE FUTURE, FOR INJURY, ILLNESS, DEATH OR DAMAGE RESULTING FROM MY ROLE AS FOSTER WITH RESCUE THE UNDERDOG.

I acknowledge that I have read, fully understood, and fully agree to each point of the RTU Foster Care Agreement.
